Items required for registration include:
1. Completed Registration Packet
2. Proof of Residence
3. Birth Certificate
4. Certificate of Immunization (Form 3231) (If the child is 4 at the time of registration, you must have a copy of the shot record.)
5. Social Security Cards (or proof of application for social security card)
6. Category 1 Documents: (Ex. Food Stamp Card, Medicaid, SSI or other federal assistance)
DAFFODIL Pre-K will offer registration packets for the 2019-2020 school year January 14 - 25, 2019.
